Note: This form is an interactive PDF. To
complete the entire form with signatures,
the user must have the full version of Adobe
Acrobat, not just Acrobat Reader. The form
can be completed without signatures,
printed, and signed manually by users of
Acrobat Reader. Signatures are required
on all applications. A current
Curriculum Vita or résumé must accompany
the form, along with a copy of recent
research from a juried or peer reviewed
publication. MTI encourages electronic
submission, if all required signatures and
documents are included.

- MTI Invoice
This form is for all vendors,
including editors, non-CSU faculty serving
as RAs or peer reviewers, and others billing
us who are not being paid as SJSU or CSU
employees who are in the payroll system and
use timesheets. If the individual or
business has a personal invoice form, they
are welcome to use that form, as long as it
allows for the same level of detail and
includes the information we need.

Detail Sheet
Independent Contractors: The Invoice Detail
Sheet must be completed and submitted with
each invoice. Claims for reimbursement
should also be detailed on the sheet to back
up the total claimed on the invoice. Attach
copies of receipts.
CSU Faculty: The Invoice Detail Sheet must
be completed and submitted with each
timesheet. Claims for reimbursement should
be detailed on the sheet; attach original
receipts.

Research in Journals
MTI strongly encourages publication of
articles based on the research we sponsor.
To assist authors in identifying juried
journals that might be interested in such
articles, we compiled a spreadsheet with
helpful information. The work was completed
in the spring of 2005. Please let us know at

- PDF

4.0+ to view and print documents. This is
free software available at http://www.adobe.com.
- Interactive
PDF files require Adobe Acrobat 5.0+,
available from Adobe. Acrobat Reader is not
sufficient to utilize the full range of
interactive features. Signed interactive
forms can be e-mailed to MTI.
- Excel
files can be saved to user’s hard drive
for completion using Excel, or can be
printed and completed manually. Electronic
forms are encouraged, and can be delivered
by e-mail.
